QCCPP DEA Incident Reporting Form 

Compliance with DEA rules in long-term care and in hospice is delaying treatment for patients, leaving them suffering in pain.   The Quality Care Coalition for Patients in Pain (QCCPP) [http://www.ascp.com/advocacy/qccpp/ ] is a coalition of over 100 physicians, nurses, pharmacists, other clinicians and consumer advocates that is working to convince members of Congress to change these rules.  We need your help to document how patients are being harmed.  Please help us by completing the attached DEA incident reporting form.  These incidents will be used to educate members of Congress and impress upon them the need to change the law.  Thank you for your help.  If you have any questions, please email gina@nadona.org.
